PERSONALIZED REMOTE TRAINING
Subscription for Wellness Coaching & PhysiApp training
EACH OPTION INCLUDES:
1. Exercise Programing - your own personal workouts (daily – goal dependent), tailored to your
needs -w/ video of each exercise & detailed written description.
2. 30-minute video/telehealth session, includes:
a. Review previous week(s) – how did the program go, how do you feel?
b. Cover details of upcoming week – schedule, life changes, etc.
c. Demonstrate and supervise exercises.
d. Address any new issue’s
e. Program adjustments and new exercises.
3. Additional Cardiovascular routine (goal dependent).
4. Wellness reminders and messaging.
5. Anytime questions, answers and adjustments to programming.
6. Increased opportunity to improve wellness as a whole; sleep, hydration, nutrition, education,
awareness, stress reduction, CONSISTENCY, etc.
7. I will be expecting feedback from you – you will be accountable.
8. Complete your routine and log your results when it fits your schedule.
9. Consults also help keep you on track and make any necessary changes to plan.
10. Loan of fitness equipment if needed (based on availability).
Option 1 Includes: 1 Telehealth sessions per month.
Option 2 Includes: 2 Telehealth sessions per month.
Option 3 Includes: 4 Telehealth sessions per month.

Remote Direct Online Personal Training
1. Private sessions are held via Telehealth or Facetime.
2. Each session is one hour- online – made by appointment.
a. Cover how you did after last session and current status
b. Perform exercises at this time (online), just as if face to face.
c. Receive motivation, physical correction and pacing with verbal and visual cueing.
3. Additional Cardiovascular routine (goal dependent).
4. Anytime questions and answers through email, text or phone.
5. I will be meeting with you for a scheduled appointment – you will be accountable.
6.This will work for those who prefer direct supervision. No reporting of your workouts is required.
7. Loan of fitness equipment if needed (based on availability).
Option 1 Includes: Per session, for ongoing training.
Option 1 Includes: Per session, for 6 or fewer sessions.
